Important Vaccination Information!

Students must provide Health Services with the immunization information prior to registering for classes. This includes immunization information on Meningitis, which was not previously required. Students who are not compliant with this requirement will be restricted from enrolling in more than 5 credit hours and/or restricted from class attendance. Please visit our immunizations page for more information.

Health Services is a student support office responsible for the verification of immunization information, physical eligibility of intercollegiate athletes, compliance with Public Health Law for students enrolled in the health care professions, injury and illness assessment and referral as needed and the provision of on-campus emergency care. Health Services does not provide primary care for students. Services are covered by a $19 Health Fee each semester, as the services rendered are not covered through insurance. The Health Services Office is staffed by a secretary and a registered nurse (nurse practitioner, supervised by a consulting physician).

Illness and Injury

  • Assessment and first aid for minor injuries, band aids, cold packs etc
  • Accident and Injury reporting
  • Over the counter medications for the relief of minor pain, colds, antacids
  • Assessment of minor illness with referral or instruction on self care as appropriate
  • Emergency response to illness and injury on campus

Records

  • Immunization Records
  • Physical Exam records – health care curricula, athletics, Children’s Learning Center and transfer
  • Medical waivers for physical education
  • Temporary disabled parking permits
  • Accident/Injury reports

Student Medical Withdrawal or Temporary Leave of Absence

If a student is on a temporary leave of absence (less than one week) due to a medical circumstance, the Office of Student Services will attempt to notify the student's instructors via electronic mail. However, it is the student's ultimate responsibility to contact each instructor to obtain missed homework and assignments and to withdraw any courses they will not be able to complete.

If a student needs to withdraw from the entire semester due to a medical circumstance, The Office of Student Services will attempt to notify the student's instructors via electronic mail. The Office of Student Services will notify Certification & Records and the instructors of the student's intent to withdraw once documentation is received.

Students can apply for a medical leave and tuition waivers/refunds by completing a Medical Leave and Tuition Waiver/Refund Application. Applications must be received by the Office of Student Services according to the following schedule:

  • Fall semester - received on or before May 31
  • Spring semester - received on or before September 1
  • All Summer terms - received on or before December 31
